Extended Family Partners
Extended family – we don’t have all the answers, so we partner with other amazing people that we consider to be part of our extended family to help make your dream life a reality. We bring high quality host homes together with individuals with developmental disabilities and support both groups with our "extended family" support.
Informal get togethers - we like to get together and enjoy our extended families' company. At Bridging Family, we have at least two get togethers a year - a summer picnic and a winter holiday party.
The Arc of Weld County (The AWC) is an advocacy organization focused on protecting the rights of individuals with intellectual and developmental disabilities. We provide no-fee individual advocacy services, community outreach programs such as events, education and training, and systems advocacy to Weld County as well as the following Northeastern Counties: Morgan, Logan, Sedgwick, Phillips, Washington and Yuma.
Envision’s Adult Case Management provides the single entry point for people requesting determination of eligibility to access developmental disabilities services in Weld County. Determination of an Intellectual or Developmental Disability (I/DD) is required prior to accessing any services through the developmental disabilities system in Colorado. Case Managers work with individuals and families to gather necessary information and to complete intake paperwork for eligibility determination. For individuals who are also applying for Medicaid Waiver services, including the Home and Community Based Services for Adults with Developmental Disabilities waiver (HCBS-DD) and the Supported Living Services waiver (SLS), Case Managers complete the required long term care assessment with the individual and caregivers. Case Managers are responsible for assuring that all individuals are afforded due process.
Envision’s Adult Case Management
Schaefer Enterprises continues to provide day program services to people with disabilities. Although we provide several different types of services to the people we serve, our main focus is and always has been to help our participants to work and earn a pay check. We provide these services in a variety of ways and in a variety of businesses.
